This is kinda tricky, shouldn't be but it is. Lets see if I can remember right.
I believe what you have to do is create the player by selecting 'create player'. After you're done editing all the details, click 'back', the computer will ask you if you're done editing the player. Click 'yes'/'ok' and then on the Management options, click on 'Add to database', and the game will you all the teams as options. Click on the team you'd like to add the player onto, and replace your custom player with an already existing player. You're done.

Wait a sec, all that should be in the manual.

If you want to do this through Player Editor (outwith the game), add to database (ingame) and then export the created player (Player Editor).
