Do not completely open the model file. For ex: simon_katich.model do not double click this file just one click and later you can see options as view,extract,replace,decompress,rebuild,exit. Click on replace and it will guide you to your system, then select the folder where you have kept simon katich's face replacement and click open. That's it its done! but be very careful while replacing.
