1. Seen this a few times
2. Not a bug. Use dive button. I quadruple checked the replay and it seems from the frames that I was in the crease but the feet or bat was not grounded in the crease. So a valid run out.
3. Seen this bug a few times. it like the program thread didn't terminate and you need to call for a run and abort immediately and you'll be safe.

Also few other misc. bugs I've seen --
1. If you take stance on leg stump and miss the ball and it goes between wickets and body, its given as a wide. Even if you attempt a shot.
2. If you call for a run and then cancel it, you cannot call for a run again for a while. The response is not good. When you cancel the run, the batsmen run to farthest end beyond crease and only then begin the again called run.
3. If you hit the ball to the stumps and the bails fly off, then when you run, you cannot get run out as the fielder only moves his hand through the bail-less stumps.
I attempted a second run, but the ball is rendered dead and the animation kicks in.
I think technically you need to uproot the stumps if the bails are off for a valid run out, but this option is not there
