So do you have to make a 'purchase' through your account (like the iTunes Store for example, where the game is just tied to your account) or is it just available to all PS+ users regardless? Like, for example, if I get a PS+ subscription starting this month, will I get Limbo (from August) for free?

You need to "purchase" it in the month it was free.
