The Custom Usertitle costs 99,999,999. If 99,999,999 is the limit and once we buy the custom usertitle, we will have 0 vCash. How are we supposed to earn vCash then? I don't have that much vCash. Just asking the question.

The only solution is someone should donate some vCash to us in that case.

When you have 0 vcash, you'll get 5,000 automatically.

There would have been a different spot to change it, possibly immediately as you bought the item. It doesn't unlock the normal username change link as they are two different systems.
