Umpires tend to make one or two glaring errors per innings maximum. I think that the third umpire should be able to challenge any decisions that he feels are massive errors from the umpire.
These decisions should be analysed for a small period of time much like line decisions and if the glaring error is as obvious as the third umpire originally thought, he can quickly call it as not out. I feel the third umpire best placed to challenge decisions because he is neutral and has the best view other than the on field umpire.
This solution will result in glaring errors being corrected and considering it will not be often and should be corrected clearly, it will not affect the flow of the game too much. People will have to accept the human error in other decisions.
Obviously not thought out massively, but I feel this should be trialed since I can find no errors in it.
