From BBC commentary I can understand that Nottinghamshire needs one point to draw with Somerset. The title will go then to Nottinghamshire, because they've won more games. A Somerset win and the title is theirs.

Actually that's written in the BBC Text commentary:

1619: Darren Pattinson finds the gap and takes the single that takes Notts to a fifth batting point. Now it's the bowlers' turn. They need three wickets in 16 overs to win their side the title. Somerset are 39-3 and have given up their run chase with nine overs left at Chester-le-Street.
